Cela semble \u00e9vident, mais il arrive souvent que les d\u00e9cisions prises par les personnes soient davantage dict\u00e9es par leur famille que par leurs propres convictions. Faites des choix auxquels vous adh\u00e9rez pleinement.<\/span><\/p>\n

Parlez-en avec vos proches, les professionnels de sant\u00e9 et votre m\u00e9decin<\/b>
\n<\/b>Faites-vous accompagner par suffisamment de personnes. Bien entendu par vos proches, mais consultez \u00e9galement les professionnels de sant\u00e9 et parlez certainement avec votre m\u00e9decin. Il peut vous expliquer en d\u00e9tail chaque traitement ou proc\u00e9dure, vous guider \u00e0 travers tous les aspects de l\u2019accompagnement de la fin de vie et vous aider \u00e0 prendre des d\u00e9cisions.<\/span><\/p>\n

Choisissez une personne de confiance<\/b>
\n<\/b>Qui pourra parler en votre nom si vous n\u2019en \u00eates plus capable ? D\u00e9terminez \u00e0 l\u2019avance le nom et les coordonn\u00e9es d\u2019une personne de confiance : cette personne deviendra alors votre repr\u00e9sentant au moment o\u00f9 vous ne serez plus en mesure de d\u00e9cider vous-m\u00eame.<\/span><\/p>\n

\u00c9tablissez vos d\u00e9clarations de volont\u00e9<\/b>
\n<\/b>1) Demandez \u00e0 votre pharmacien le <\/span>plan LEIF gratuit<\/b>. Vous y trouverez les diff\u00e9rentes d\u00e9clarations de volont\u00e9 ainsi que des explications d\u00e9taill\u00e9es. Vous pouvez \u00e9galement t\u00e9l\u00e9charger s\u00e9par\u00e9ment les mod\u00e8les de documents sur<\/span> www.leif.be<\/span><\/a>.<\/span>
\n<\/span>2) Faites-vous accompagner par un m\u00e9decin lors de la r\u00e9daction de vos d\u00e9clarations de volont\u00e9. Ou contactez un professionnel de l\u2019aide via la ligne LEIF : 078 15 11 55.<\/span>
\n<\/span>3) Conservez simplement les documents remplis chez vous. Vous pouvez \u00e9galement faire conserver une copie par votre personne de confiance. Les d\u00e9clarations concernant l\u2019euthanasie, le don d\u2019organes et la s\u00e9pulture peuvent \u00e9galement \u00eatre enregistr\u00e9es aupr\u00e8s de la commune. Ce n\u2019est pas obligatoire.<\/span>
\n<\/span>4) Demandez votre <\/span>carte LEIF gratuite<\/b> sur<\/span> leifkaart.nu<\/span><\/a>. Cette carte mentionne clairement vos d\u00e9clarations de volont\u00e9 ainsi que les coordonn\u00e9es de votre personne de confiance. Vous la gardez dans votre portefeuille.<\/span><\/p>\n

\n<\/b>Sachez \u00e9galement que vous ne devez pas tout d\u00e9cider en une seule fois. R\u00e9fl\u00e9chir \u00e0 sa fin de vie est un processus.
