Je vois les choses de la m\u00eame mani\u00e8re dans la prise en charge des probl\u00e8mes psychiques. C\u2019est une autre r\u00e8gle empirique de la m\u00e9decine g\u00e9n\u00e9rale.<\/span><\/p>\n

J\u2019appelle \u00ab software \u00bb tout ce qui r\u00e9sulte d\u2019exp\u00e9riences traumatiques du pass\u00e9, d\u2019un chagrin non r\u00e9solu, d\u2019un deuil interminable, de diverses formes de remords et de ranc\u0153ur, de sentiments de culpabilit\u00e9, de col\u00e8re face \u00e0 l\u2019injustice, etc. Ces r\u00e9cits sont biographiques et il s\u2019agit ici principalement d\u2019un travail d\u2019\u00e9laboration.<\/span><\/p>\n

En pratique, ce sont tous des cas typiques qui rel\u00e8vent du domaine du psychologue, du moins dans le cadre que j\u2019applique inconsciemment chaque jour. Or, dans ce pays, nous manquons cruellement de psychologues. Ou plut\u00f4t non. Ils sont bien l\u00e0, mais ils ne sont pas accessibles, parce que leurs services ne sont pas rembours\u00e9s, ou \u00e0 peine, par la caisse d\u2019assurance maladie.<\/span><\/p>\n

La psychiatrie regorge de diagnostics. La terminologie utilis\u00e9e \u00e0 cet effet est assez complexe. Au risque de perdre quelques nuances, je l\u2019ai r\u00e9partie ici de mani\u00e8re simple. Les \u00ab probl\u00e8mes de software \u00bb correspondent, dans ma terminologie personnelle et simplifi\u00e9e, aux n\u00e9vroses, par opposition aux d\u00e9fauts de hardware, parmi lesquels figurent les psychoses.<\/span><\/p>\n

Il s\u2019agit une fois encore de r\u00e8gles empiriques caricaturales, qui constituent n\u00e9anmoins un rep\u00e8re pour \u00e9valuer les troubles du comportement et de l\u2019exp\u00e9rience v\u00e9cue. Nous avons tous nos strat\u00e9gies pour faire face \u00e0 une r\u00e9alit\u00e9 hostile. Nous avons tous nos peurs et nous traversons tous, \u00e0 un moment ou \u00e0 un autre, des p\u00e9riodes o\u00f9 nous flanchons. Il faut une certaine exp\u00e9rience pour sentir quand les limites du supportable sont atteintes.<\/span><\/p>\n

Parfois, cela devient trop lourd et nous devons alors pouvoir nous raccrocher \u00e0 ce qui, en d\u00e9finitive, nous permet de tenir debout. Pour ma part, c\u2019est le souci que nous avons les uns des autres. C\u2019est le fondement de ce que je veux faire. La mani\u00e8re de proc\u00e9der d\u00e9pend de l\u2019exp\u00e9rience et des connaissances que l\u2019on acquiert. Celles-ci reposent sur une formation qui, dans mon cas comme dans celui de nombreuses g\u00e9n\u00e9rations de m\u00e9decins apr\u00e8s moi, reste insuffisante dans le domaine de la sant\u00e9 mentale.<\/span><\/p>\n

Au fil de ma carri\u00e8re, j\u2019ai d\u00fb apprendre \u00e0 y voir clair dans la jungle des manifestations de l\u2019\u00e2me de toutes sortes que nous rencontrons dans la pratique. On apprend cela progressivement au cours de sa carri\u00e8re. Il suffit de regarder ce que l\u2019on peut faire, et parfois d\u2019apprendre \u00e0 accepter qu\u2019il existe aussi des situations qui \u00e9chappent \u00e0 notre influence th\u00e9rapeutique. On ne gagne pas toujours et, parfois, quelqu\u2019un se perd en chemin.<\/span><\/p>\n
