Le trouble de stress post-traumatique (TSPT) est un trouble psychique qui, dans l\u2019ancienne DSM-IV, \u00e9tait class\u00e9 parmi les troubles anxieux. Dans l\u2019actuelle DSM-5, le trouble figure dans un nouveau chapitre, celui des troubles li\u00e9s aux traumatismes et aux facteurs de stress.<\/span><\/p>\n

Ce trouble est la cons\u00e9quence d\u2019\u00e9v\u00e9nements dramatiques au cours desquels la vie ou l\u2019int\u00e9grit\u00e9 physique sont mises en jeu et qui constituent le traumatisme. Prenons la guerre ou d\u2019autres conflits arm\u00e9s. De la Premi\u00e8re Guerre mondiale, nous connaissons le syndrome du <\/span>shell shock<\/span><\/i>, et de la Seconde Guerre mondiale, la <\/span>battle fatigue<\/span><\/i> et le syndrome des camps de concentration.<\/span><\/p>\n

Dans les ann\u00e9es soixante et soixante-dix, et jusqu\u2019\u00e0 aujourd\u2019hui, il y a eu aux \u00c9tats-Unis le syndrome des v\u00e9t\u00e9rans du Vietnam, et plus r\u00e9cemment le syndrome des Balkans chez nos propres anciens combattants. Les violences graves, l\u2019agression ou les menaces, l\u2019attaque, le vol ou la torture, mais aussi l\u2019exercice du pouvoir dans la sph\u00e8re domestique, comme les actes p\u00e9dophiles ou l\u2019inceste, peuvent \u00e9galement d\u00e9clencher ce syndrome.<\/span><\/p>\n

Apr\u00e8s un accouchement traumatique, on peut d\u00e9velopper un TSPT post-partum, et apr\u00e8s la perte d\u2019un \u00eatre cher, on est confront\u00e9 \u00e0 un processus de deuil, qui constitue une forme distincte. Les victimes de harc\u00e8lement prolong\u00e9 et s\u00e9v\u00e8re, ainsi que les t\u00e9moins de telles situations qui ne sont pas intervenus et d\u00e9veloppent de ce fait un sentiment de culpabilit\u00e9, peuvent eux aussi \u00eatre concern\u00e9s.<\/span><\/p>\n

Une \u00e9num\u00e9ration pour le moins disparate. Le sympt\u00f4me caract\u00e9ristique est la reviviscence r\u00e9p\u00e9t\u00e9e et r\u00e9currente, sous forme de cauchemars ou de flashbacks qui rejouent sans cesse l\u2019\u00e9v\u00e9nement \u00e9pouvantable comme un film devant l\u2019\u0153il de l\u2019esprit. La personne concern\u00e9e d\u00e9veloppe toutes sortes de strat\u00e9gies pour faire face \u00e0 ce souvenir douloureux, en l\u2019\u00e9vitant ou en essayant de le neutraliser \u00e9motionnellement.<\/span><\/p>\n

Ce n\u2019est pas aussi simple qu\u2019il y para\u00eet, car ce que l\u2019on chasse par la porte revient par la fen\u00eatre. \u00c0 moins de se fermer compl\u00e8tement et de sombrer dans un \u00e9tat proche de celui d\u2019un zombie.<\/span><\/p>\n

On ne parle de TSPT que lorsque les sympt\u00f4mes durent plus d\u2019un mois. Lorsqu\u2019ils durent moins d\u2019un mois, on parle de trouble de stress aigu. Le TSPT peut \u00eatre gu\u00e9ri ou am\u00e9lior\u00e9 gr\u00e2ce \u00e0 un traitement. Il peut parfois aussi dispara\u00eetre spontan\u00e9ment, mais cela ne se fait pas toujours de soi-m\u00eame.<\/span><\/p>\n

Dans son \u00e9volution d\u00e9favorable, le TSPT peut entra\u00eener des sympt\u00f4mes appartenant au registre d\u00e9pressif : abattement, irritabilit\u00e9, m\u00e9fiance, irritation, r\u00e9actions de frayeur intenses et surtout troubles du sommeil.<\/span><\/p>\n","protected":false},"excerpt":{"rendered":"
