Philosophiquement, il est tr\u00e8s difficile de d\u00e9terminer ce qui est infond\u00e9 ou faux. Dans la pratique clinique, il m\u2019arrive de me retrouver, debout ou assis, face \u00e0 face avec quelqu\u2019un qui per\u00e7oit le monde autrement que moi. Automatiquement, je prends moi-m\u00eame pour mesure de la v\u00e9rit\u00e9. En tant qu\u2019aidant, je ne peux alors faire autrement que de mettre ma propre personnalit\u00e9 dans la balance. Pour cela, il faut soi-m\u00eame \u00eatre un tant soit peu solide psychiquement.<\/span><\/p>\n

Le psychotique \u00e9prouve les choses autrement que la plupart d\u2019entre nous. Les co\u00efncidences acqui\u00e8rent une signification nouvelle et \u00e9nigmatique. Partout apparaissent des \u00ab signes \u00bb d\u2019une mission ou d\u2019une vocation provenant d\u2019une puissance myst\u00e9rieuse. Cela s\u2019accompagne parfois d\u2019hallucinations, au cours desquelles une ou plusieurs voix s\u2019adressent \u00e0 toi.<\/span><\/p>\n

La personne elle-m\u00eame a l\u2019impression que cela vient de l\u2019ext\u00e9rieur et y per\u00e7oit une certaine coh\u00e9rence, un syst\u00e8me d\u00e9lirant qui, int\u00e9rieurement, est parfaitement logique, mais qui ne peut \u00eatre concili\u00e9 avec \u00ab nos \u00bb conceptions ou notre logique. Si la personne concern\u00e9e n\u2019est pas dispos\u00e9e \u00e0 confronter son point de vue au monde que nous connaissons, tu as affaire \u00e0 quelqu\u2019un qui d\u00e9lire.<\/span><\/p>\n

Avec toutes les r\u00e9serves qui s\u2019imposent, au cas o\u00f9 ce serait moi qui serais en train de d\u00e9lirer. C\u2019est encore autre chose que d\u2019halluciner. Ce n\u2019est pas un tr\u00e8s joli mot. Les hallucinations sont des impressions apparemment sensorielles. C\u2019est quelque chose que tu entends ou que tu vois, ou que tu crois entendre ou voir. Ou sentir. Quelque chose qui n\u2019est pas l\u00e0, mais comment sais-tu qu\u2019il n\u2019y a rien ?<\/span><\/p>\n

Les hallucinations faisaient partie des sympt\u00f4mes obligatoires de la psychose dans l\u2019ancienne conception, fortement influenc\u00e9e par la psychanalyse. \u00c0 strictement parler, l\u2019hallucination est autre chose qu\u2019un d\u00e9lire. Quelqu\u2019un qui pense \u00eatre Napol\u00e9on est dans le d\u00e9lire. Quelqu\u2019un qui affirme s\u00e9rieusement avoir entendu ou rencontr\u00e9 Napol\u00e9on hallucine.<\/span><\/p>\n

La question essentielle est \u00e9videmment : comment g\u00e9rer cela ? Je ne vais pas en discuter avec lui ou elle, car cela ne m\u00e8nerait \u00e0 rien. Quelqu\u2019un qui d\u00e9lire ne peut \u00eatre convaincu de la r\u00e9alit\u00e9 des faits par des arguments rationnels. Le patient peut penser qu\u2019il est Napol\u00e9on, mais moi, en tant que m\u00e9decin g\u00e9n\u00e9raliste, je peux consid\u00e9rer que ce n\u2019est pas le cas. J\u2019appelle son id\u00e9e un d\u00e9lire, mais ma propre id\u00e9e constitue la v\u00e9rit\u00e9 objective, simplement parce que je suis l\u2019aidant.<\/span><\/p>\n

En tant que m\u00e9decin g\u00e9n\u00e9raliste, tu ne peux parler avec le psychotique que si tu entres, dans une certaine mesure, dans son d\u00e9lire.
