On peut grosso modo distinguer deux formes de psychose. Les deux extr\u00eames sont la psychose affective et la psychose schizophr\u00e9nique.<\/span><\/p>\n

Dans la psychose affective, les troubles de l\u2019humeur occupent une place centrale : l\u2019humeur fluctue et peut \u00eatre gravement perturb\u00e9e. La forme la plus connue est le trouble <\/span>bipolaire<\/span><\/i>, avec ses extr\u00eames caract\u00e9ristiques de moments de profonde d\u00e9pression et de phases d\u2019exaltation d\u00e9bordante. Nous l\u2019aborderons s\u00e9par\u00e9ment dans un autre chapitre.<\/span><\/p>\n

Dans la psychose schizophr\u00e9nique, la pens\u00e9e est beaucoup plus perturb\u00e9e et des id\u00e9es d\u00e9lirantes et des hallucinations apparaissent, parfois visuelles, lorsque la personne pense voir certaines choses, mais plus souvent auditives : elle entend des voix. Il existe \u00e9galement une forme mixte, la psychose schizo-affective. La distinction n\u2019est pas toujours facile \u00e0 \u00e9tablir, et nous laissons cela au psychiatre.<\/span><\/p>\n

La maladie mentale peut se manifester sous forme de crises. Dans les moments aigus, nous devons intervenir, en tant que m\u00e9decins g\u00e9n\u00e9ralistes, avec des m\u00e9dicaments psychiatriques puissants. Lorsqu\u2019une pouss\u00e9e psychotique s\u00e9v\u00e8re survient, par exemple, parce que nous ne pouvons pas attendre le psychiatre. Ces m\u00e9dicaments sont souvent extr\u00eamement efficaces, mais le patient doit accepter de les prendre. Et cela ne r\u00e9ussit pas toujours.<\/span><\/p>\n

Une pouss\u00e9e psychotique est souvent une exp\u00e9rience bouleversante qui soul\u00e8ve de nombreuses questions sur le sens de la vie et la place que l\u2019on occupe dans la soci\u00e9t\u00e9. Qu\u2019en est-il des relations, de la sexualit\u00e9, de l\u2019amour et de la mort ? La psychose remet tout en question, jusqu\u2019\u00e0 l\u2019origine de sa propre existence, \u00e0 la signification du monde et \u00e0 la diff\u00e9rence entre d\u00e9lire et v\u00e9rit\u00e9.<\/span><\/p>\n

La crise psychotique peut sembler dramatique, mena\u00e7ante ou terrifiante, mais l\u2019inverse peut \u00e9galement se produire : elle devient alors l\u2019exp\u00e9rience la plus heureuse et la plus profonde de votre vie. Certains \u00e9prouvent alors un sentiment de r\u00e9v\u00e9lation bouleversant, qui peut tendre vers l\u2019extase et ouvrir une dimension spirituelle, au point que l\u2019on puisse parler de mystique.<\/span><\/p>\n

Lorsque des exp\u00e9riences m\u00e9taphysiques surviennent sous l\u2019influence de drogues, on peut naturellement se demander : \u00e9tait-ce d\u00fb aux substances consomm\u00e9es, ou bien s\u2019est-il r\u00e9ellement produit quelque chose d\u2019extraordinaire ? Nous pourrions en parler longuement, mais dans la pratique, il faut intervenir dans un contexte de crise, parce que quelque chose a d\u00e9rap\u00e9.<\/span><\/p>\n

Il faut alors commencer par r\u00e9soudre l\u2019urgence aigu\u00eb et examiner ensuite ce qui se passe apr\u00e8s la crise. La psychose perturbe la vie quotidienne et entrave l\u2019int\u00e9gration dans la soci\u00e9t\u00e9.<\/span><\/p>\n","protected":false},"excerpt":{"rendered":"
