Depuis des dizaines d\u2019ann\u00e9es, la litt\u00e9rature est le th\u00e9\u00e2tre d\u2019une querelle de mots autour de la question de savoir si le cannabis joue ou non un r\u00f4le causal dans l\u2019apparition d\u2019une psychose. On trouve de nombreuses \u00e9tudes qui \u00e9tablissent effectivement ce lien, notamment aux \u00c9tats-Unis. En 2017, on estimait \u00e0 35 millions le nombre d\u2019Am\u00e9ricains qui consommaient au moins une fois par mois les diff\u00e9rentes formes de pr\u00e9sentation du cannabis, comme le haschisch et la marijuana.<\/span><\/p>\n

Le cannabis est la drogue r\u00e9cr\u00e9ative la plus largement r\u00e9pandue dans nos soci\u00e9t\u00e9s occidentales et bien au-del\u00e0. Beaucoup d\u2019encre a coul\u00e9 sur l\u2019existence d\u2019un lien causal entre sa consommation et l\u2019apparition d\u2019une psychose, mais la preuve ultime n\u2019en a jamais \u00e9t\u00e9 apport\u00e9e. La cause de la psychose se situe dans le cerveau et tout porte \u00e0 croire que l\u2019on na\u00eet avec cette pr\u00e9disposition, ou qu\u2019on ne l\u2019a pas. Avec l\u2019\u00e9tat actuel de la science, on ne peut plus modifier ses chromosomes.<\/span><\/p>\n

Il est toutefois vrai que, chez certains jeunes, car ce sont g\u00e9n\u00e9ralement de jeunes adultes qui exp\u00e9rimentent ces substances, la consommation de cannabis peut d\u00e9clencher une pouss\u00e9e psychotique. Tant\u00f4t elle est hilarante et sans s\u00e9quelles durables, parfois m\u00eame particuli\u00e8rement spirituelle, mais elle peut aussi \u00eatre extr\u00eamement angoissante et terrifiante. En tant que m\u00e9decin g\u00e9n\u00e9raliste, j\u2019en ai suffisamment vu pour que cela m\u2019inqui\u00e8te.<\/span><\/p>\n

Le march\u00e9 propose des produits \u00e0 concentration toujours plus \u00e9lev\u00e9e, qui seraient encore beaucoup plus susceptibles de d\u00e9clencher une psychose que ceux que nous connaissions auparavant. Plusieurs \u00e9tudes montrent du doigt cette tendance \u00e0 des produits toujours plus puissants, qui, au fond, ne change pas grand-chose au probl\u00e8me.<\/span><\/p>\n

Nous ne souhaitons \u00e0 personne de conna\u00eetre une pouss\u00e9e psychotique, et il faut prendre cela au s\u00e9rieux, mais tout porte \u00e0 croire que cet effet psychog\u00e8ne du cannabis r\u00e9cr\u00e9atif ne concerne que les personnes vuln\u00e9rables. Cela arrive relativement rarement, mais, dans leur cas, les cons\u00e9quences peuvent \u00eatre totalement d\u00e9sastreuses. Je ne conseillerais pas \u00e0 des adolescents psychotiques de se mettre au cannabis, mais il m\u2019arrive souvent de donner ce conseil trop tard.<\/span><\/p>\n

Il existe \u00e9galement des articles qui soulignent les effets b\u00e9n\u00e9fiques de certains extraits de cannabis dans le traitement de certaines formes de psychose. La question n\u2019est pas encore tranch\u00e9e. D\u2019autres substances susceptibles de provoquer des sympt\u00f4mes psychotiques chez les personnes vuln\u00e9rables sont l\u2019alcool, les d\u00e9riv\u00e9s de l\u2019opium, les tranquillisants, la coca\u00efne, les amph\u00e9tamines et, bien s\u00fbr, en particulier, les hallucinog\u00e8nes comme le LSD. Le cannabis domine largement, parce qu\u2019il est tellement consomm\u00e9, mais aussi parce que, chez une petite minorit\u00e9 d\u2019utilisateurs, son effet hallucinog\u00e8ne peut \u00eatre particuli\u00e8rement marqu\u00e9.<\/span><\/p>\n","protected":false},"excerpt":{"rendered":"
