Une psychose peut avoir un effet d\u00e9vastateur sur l\u2019int\u00e9gration sociale et g\u00e2cher le cours d\u2019une vie, parce que l\u2019on n\u2019arrive plus \u00e0 suivre les autres. Cela suscite ressentiment et frustration et peut conduire \u00e0 un sc\u00e9nario d\u2019\u00e9chec o\u00f9 tout va mal et o\u00f9 rien ne r\u00e9ussit.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Cela peut entra\u00eener une d\u00e9pression suppl\u00e9mentaire, disons une d\u00e9pression r\u00e9actionnelle ou exog\u00e8ne provoqu\u00e9e par les revers de l\u2019existence. Il n\u2019est pas aussi facile qu\u2019on pourrait le croire d\u2019en parler avec quelqu\u2019un. Les troubles psychiques restent frapp\u00e9s d\u2019un grand tabou et l\u2019ignorance \u00e0 leur sujet est stup\u00e9fiante, ce qui laisse libre cours \u00e0 toutes sortes de pr\u00e9jug\u00e9s et de conceptions erron\u00e9es.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Beaucoup de gens ignorent eux-m\u00eames qu\u2019ils souffrent d\u2019une psychose. Je rencontre encore assez souvent des patients psychotiques qui, apr\u00e8s \u00eatre pass\u00e9s par toutes sortes de structures de soins, n\u2019ont pas la moindre id\u00e9e de leur diagnostic, qu\u2019il s\u2019agisse de formes davantage bipolaires ou davantage schizophr\u00e9niques.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Dans le cas des patients psychiatriques, celui-ci n\u2019est souvent pas communiqu\u00e9. Certainement du c\u00f4t\u00e9 francophone, car les choses s\u2019y passent parfois de mani\u00e8re totalement diff\u00e9rente. Cela m\u00e9riterait \u00e0 lui seul un livre. \u00c0 Bruxelles, je suis bien plac\u00e9 pour observer les diff\u00e9rences de terminologie, mais aussi de philosophie, entre les deux communaut\u00e9s linguistiques en mati\u00e8re de soins de sant\u00e9 mentale.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Il est tr\u00e8s int\u00e9ressant de comparer tout cela, mais dans la pratique, les personnes psychotiques restent souvent laiss\u00e9es pour compte lorsqu\u2019il s\u2019agit de leur fournir une information correcte sur leur maladie. En tant que m\u00e9decin g\u00e9n\u00e9raliste, je donne g\u00e9n\u00e9ralement le nom de la maladie. Du moins, j\u2019exprime mon hypoth\u00e8se afin de voir comment elle est re\u00e7ue, m\u00eame lorsqu\u2019il s\u2019agit d\u2019une psychose. Car je consid\u00e8re qu\u2019un diagnostic est une cl\u00e9 qui ouvre la serrure de la connaissance.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">C\u2019est toute l\u2019id\u00e9e qui sous-tend la m\u00e9decine : nous pouvons reconna\u00eetre et nommer des tableaux cliniques, auxquels correspond alors une conduite \u00e0 tenir appropri\u00e9e. Il s\u2019agit de connaissances, d\u2019exp\u00e9rience et de science. Je ne les mets pas seulement au service du public, je dois aussi en transmettre une partie. C\u2019est souvent l\u00e0 que quelque chose se d\u00e9r\u00e8gle dans la m\u00e9decine classique. Le savoir, c\u2019est le pouvoir, et la transmission du savoir est un transfert de pouvoir.
